We are pleased to announce the Call for Submissions for the Female Care Provider of Excellence Award. Female Care Providers throughout Southern California are continually setting standards of excellence, demonstrating superior achievements and delivering outstanding care to residents. This award was established to recognize the hard work, unselfish acts, and unwavering commitment of female care providers to caring for our elderly.
Description:
This award shall be presented to an exceptional female caregiver, owner, or operator of a residential care facility for the elderly or adult residential facility whose dedication to her residents and outstanding service to her community is demonstrate through her actions.
Award Highlights:
The honoree will be announced at the Female Care Providers 2019 conference on October 4th.
To Participate:
Complete and submit the application and letter no later than September 25, 2019
Review and Selection Process:
All entries received by September 25th will be reviewed by a selection committee comprised of senior care professionals in various areas.
Entry Format
Please share the reasons you believe this person is deserving of the title 2018-2019 Female Care Provider of Excellence and submit a letter of nomination by answering the following questions in your nomination letter.
- What accomplishments above and beyond her job description validate an exceptional commitment to residents and other care providers?
- What achievements has your nominee made, personally and/or professionally, that are evidence of her qualification for the Female Care Provider of Excellence Award?
- What qualities does your nominee possess that make her an effective and compassionate caregiver?
- Anything else you’d like to share about this female care provider?
